If you are ready to subscribe with ConnectCare for yourself, a friend, family member or a patient/client, please submit the information below and a ConnectCare representative with get back to you. If you prefer, call us directly at 519•685•4550, or toll free at 1•888•298•6116.

After answering important subscriber intake questions, a Representative will be scheduled to contact you within 24 hours during regular business hours Monday - Friday. When it is convenient, an appointment will be scheduled with a qualified volunteer installer in your area, to install and test your equipment, and answer any questions that you may have related to your new ConectCare service or products.

... and remember, no long term contract is required!


Subscriber Intake Questions ...

During the intake process, the following information will be requested and kept on file:

  • Subscriber:
    • Name
    • Address (including Postal Code)
    • Phone type(s), number(s) & service provider(s):
      • regular land line, cell phone, both?
      • Rogers, Bell, other?
    • Medical Conditions
    • Medications - Where are they kept? Allergies to medications?
    • Vision - Glasses? Contact Lens?
    • Hearing Conditions - Hearing Aids?
    • Mobility Issues? Cane? Walker? Wheel Chair? How often do you leave your home (daily, weekly, infrequently)?
  • Next of Kin:
    • Name
    • Relationship to Subscriber - Spouse? Daughter? Son? Sibling? Other Relative?
    • Contact Phone Numbers
  • List of Responders
  • How did you hear about ConnectCare?
